Two Recent Full Body photographs.

 Photocopy of age proof documents ( 10th marksheet / licence copy/   passport etc.)

Rules and Regulation form duly signed by contestants.

Judges information sheet.

Health information form.

Residential Proof ( 6 months previous domicile).
